File your claim via Email.

Follow the instructions below.

Step 1.

Download and fill out the Life Insurance Claim Form, including the Release and Subrogation Form.
Prepare the documents listed in the Life Insurance Claim Form.

Step 2.

Send the documents to ygccustomerservice@ygc.com.ph.
Use the following format for the email subject:
- Policy Number | Policy Type | Claim Type
  - Ex. 123344 | My Family Shield | Daily Hospital Income Benefit Claim
